The products and services provided by Preferred Homecare are designed to make it easier to care for patients in their own homes. We offer:
- Team Coordination with physicians as team leaders in partnership with nurses, discharge planners and insurers.
- Recommendation of appropriate therapies, equipment, and care plans.
- Care Coordination through coordination with Preferred Homecare's team of registered nurses, registered dietitians, and clinical pharmacists.
- Monitoring and evaluation of responses to treatment.
- Regular updates on patient progress.
- Specialized training and education for patients and caregivers.
- Delivery of all drugs, equipment, and supplies, day or night, all year long.
